Parents: We have gone to an online system called Rank One for our athletic forms this year. You will need to go to the Rank One website/App to fill them out. This can be done from your phone or computer. (There is a rank one App that you can download. )

Follow the directions below. 
1. Go to the Rank One website listed below these intructions
2. Choose the Parents Click Here tab
3. Click Online Forms
4. Select Oklahoma
5. Select Warner Public Schools
6. Add your children using their name and school ID# (this is their lunch number - a six digit number)
7. Scroll down and click create a new account 
8. You will need to select the sports that they will be participating in.
9. Once you have added your children, scroll down the page to the student profile and click the little arrow on the right to access the forms. If they did not get their physical at the school you will need to upload a copy of their physical.
